Kindred Community is my 3rd case management team and I cannot fault the team I have! Honestly the best case management team I could ask for. Everything is always organised, Kat and her team have been brilliant picking up the pieces that wasn't done from my old case manger, and handling everything (old or new) that has been thrown at them.

She worked with the client on a daily basis during some very difficult times for my client (drug use, accommodation issues, care troubles and disengagement in therapies). Sarah's support to the client has been invaluable and continues to be so. Kat has been outstanding throughout her role as my relatives' Case Manager. Kat has been proactive throughout in arranging an MDT to support their rehabilitation. She has advocated really effectively for my relative when transferring from hospital to a private care home.
Kat is very good at communicating with the family how things are progressing with treatment and organising regular MDT meetings. The family have felt very included in all decisions which are taken in my relatives' care.

It has been my experience that you treat each client with care and compassion as individuals and support them to the best of your ability. You ask for clinical guidance when needed to support your case management role. You are keen to learn about your clients clinical (cognitive and neurobehavioural) presentations and needs, often going the extra mile to support them to be independent, with the necessary support, risk analysis, and case management in place. We have some interesting, thought provoking, and inspiring conversations too.

I have worked with Catherine Baker on several cases in the past couple of years. She is skilled in building strong relationships whilst maintaining a calm, empathetic and patient manner with even the most challenging of clients and their families.
