Kindred Community is my 3rd case management team and I cannot fault the team I have! Honestly the best case management team I could ask for. Everything is always organised, Kat and her team have been brilliant picking up the pieces that wasn't done from my old case manger, and handling everything (old or new) that has been thrown at them.

She worked with the client on a daily basis during some very difficult times for my client (drug use, accommodation issues, care troubles and disengagement in therapies). Kat has been outstanding throughout her role as my relatives' Case Manager. Kat has been proactive throughout in arranging an MDT to support their rehabilitation. She has advocated really effectively for my relative when transferring from hospital to a private care home.
Kat is very good at communicating with the family how things are progressing with treatment and organising regular MDT meetings. The family have felt very included in all decisions which are taken in my relatives' care.
