With many years of collective experience, Kindred Community Ltd incorporates a variety and ever evolving use of tailormade strategies and approaches into their practice as healthcare professionals to ensure we meet the client’s needs in a way that suits them. We believe working in a holistic, transparent manner is best for all and enhances the case management relationship. The client is firmly at the centre of all we do, working collaboratively with family, support staff, therapists, legal and finance teams which all make up the wider network. Working closely, flexibly and in a reflective manner means we can support our clients and their wider network to navigate together challenges to overcome and goals to be accomplished.

Input from us as your case manager can come in many different forms; face to face, telephone, online meetings, through multi-disciplinary meetings and of course we will make sure all the administration, liaison and referrals are done by us too. All our work is itemised, available to our client and invoiced monthly. We are CQC registered for personal care and treatment of disease disorder and injury domains for regulated activity, as governance for professional good practice and ensuring excellent quality of provision.

Our case managers work from a relational approach. The relationship between the client and Case Manager as a vehicle to empower, encourage and drive towards rehabilitation goals and personal dreams.
